Home care for older people

This is a new guideline from NICE on homecare for older people. Most of this isn't new, or is aimed at other health professionals, but there are a couple of points of note for us.

Care Coordinator.

Nice advise considering organising a named care coordinator. This would normally be a social worker, a practitioner working for a voluntary or community sector organisation or a lead nurse. This person would be responsible for leading care planning and also for ensuring that everyone knows what they should be providing and when and also that there is regular communication between those providing care.

Home care plan.

Among other things, this will include information about the medicines needed, their purpose, the timing of administration and who to contact if there are concerns.
