Salary: £57,084 - £80,953 per annum* (up to ten sessions)

pioneering project in West London. The posts have been designed specifically to
help NHS Hammersmith and Fulham to deliver significant health improvements to
local communities.

The posts offer motivated GPs an exceptional opportunity to shape the
development of:
- The Cassidy Road Medical Centre, which is a VISION practice with a list
size of 4,000. (Job reference 659-MED-GP004)
- The Canberra Health Centre which will be a VISION paper-light practice
with a projected list size of 4,000 by 2011 and 8,000 by 2014. (Job
reference 659-MED-GP003)
Working with other leaders in the community, you will drive the redesign and
development of the primary care services and support the expanding roles of a
multidisciplinary team. You will report to the Polysystems Clinical Lead.
